About Us

Family Rootz LLC is a domestic limited liability company registered with the Indiana Secretary of State. We are a non-accredited mental health agency providing CMHW services for children ages 6 to 17 who have been identified as having a need. At Family Rootz, we come together with transparency, honesty, integrity, and a genuine desire to aid in the growth of ourselves, our employees, and the children and families we provide services to. We will train ourselves and our staff to provide services to help children and their families become the best versions of themselves.

Our Services

CMHW Respite- a short-term break for caregivers of children in the Child Mental Health Wraparound (CMHW) program

 

CMHW Habilitation- a program that provides one-on-one skill-building services for children and youth at risk. The program's goal is to help children and youth develop the skills they need to improve their quality of life, functioning, and resilience

 

Family Support and Training (FST)- a service offered through the Child Mental Health Wraparound (CMHW) program in Indiana. FST is designed to help family members and extended family supports improve their family's functioning and support the youth and family in the community, home, and school.

My name is Jason Dobbins. I was born and raised in Cleveland Ohio. I am a father of 4 with 3 bonus kids. I landed in Monticello due to job opportunities and circumstances. I’ve lived in inner cities, suburbs, and rural communities. I have true knowledge and life experiences that have equipped me with the skills needed to provide respite, HAB, and FST. I have been blessed with the opportunity to work with a child locally. I have been able to create trust connections in the boundaries needed to have a positive impact on the overall health of this child. Teaching self-confidence and physical well-being and connecting those to emotional well-being is important to the overall health of any child. I believe I have the ability to help more children attain better physical, mental, and emotional health. With the forming of Family Rootz, I plan to personally provide services and be a part of hiring and training others to provide services. 

 

My name is Brooke Johnson. I was born and raised in Indiana and have been in Monticello most of my life. I previously owned and operated Little Ras-kel’s daycares in Monticello, Indiana. Personally, and in business, I witnessed firsthand the gap in mental health services and providers that were readily available to children in the community. I’ve been involved with several children creating plans of care and helping to implement them at home and in the community. After taking a step away and realizing the true lack of services available to my own child I knew I needed to be a part of building an agency that could specialize in offering care to children. With the partnership of Family Rootz, I plan to use my past experience and knowledge to help guide children and new providers that will offer respite, HAB, and FST.

If you are interested in becoming involved with what we are doing on a more professional level please submit a resume, cover letter, and Family Rootz info sheet. (For positions other than childcare, providing proof of involvement in the direct care of children ages 6-17 with serious emotional disturbance (SED). (Respite requires 1 year and HAB and FST require 2 years). The most recent qualifying experience must be no more than three years prior to the date of application. Experience of more than eight years in the past will not be considered as qualifying). Our childcare positions could act as a training ground and experience needed for the other positions. When ready we will forward your application to the Division of Mental Health and Addiction (DMHA) for approval of your experience for those positions. After an official offer of employment, your next steps will be discussed individually depending on your position. All positions will be required to do drug testing, fingerprints, criminal check, CPR, and first aid.
